Most hospital facilities are now adopting EBP to reduce readmissions due to the Affordable Care Act and the Hospital Readmissions Reduction Program which has legislated penalties for hospital readmission within 30 days for certain diagnosis (Bowles et al., 2015). At the facility where I work we have implemented a transitional care program, discharge follow-up call 24-48hrs after discharge and use risk scores to determine who may be at risk for hospital readmissions. The EBP of providing discharge interventions such as discharge follow-up calls has proven to reduce ER visits and hospital readmissions (Costantino, et al., 2013). There are several reasons for hospital readmissions, however I have notice some patients are non-compliance with managing their care or do not understand their discharge instructions. I have made calls to patients after discharge to only find out they did not fully understand their discharge instructions as to which medications to take or which medications to stop taking. During these phone call we assess if patients understand their discharge instructions, picked up their medications, know what symptoms to look for, follow-up appointment given within 5-7 day and answer any questions. Part of the intervention is to contact the patients’ primary care physician or the pharmacist for further instructions if needed.
